Let's talk about Doxycycline induced fistula
If you lie down immediately after taking doxycycline, or takes it with too little water, the capsule or tablet can get stuck in the esophagus. Because doxycycline is acidic, it can directly damage the lining of the esophagus.
What happens:
1. Pill lodges in the esophagus → dissolves slowly against the mucosa.
2. Local acid injury → causes chemical esophagitis (burn-like injury).
3. Symptoms within hours to days:
Sudden severe chest pain (often sharp, mid-chest)
Painful swallowing (odynophagia)
Difficulty swallowing (dysphagia)
Sometimes hematemesis (vomiting blood)
4. Complications if severe or untreated:
Deep ulceration of the esophagus
Strictures (scarring that narrows the esophagus)
Rarely, a fistula (abnormal connection, e.g., to the trachea)
Prevention:
Always take doxycycline with a full glass of water (≥200 ml / ~8 oz).
Remain upright for at least 30 minutes after swallowing it.
Avoid taking it right before bedtime.
