This is to inform all 𝗳𝗼𝘂𝗿 𝗵𝘂𝗻𝗱𝗿𝗲𝗱 𝗲𝗶𝗴𝗵𝘁𝘆-𝘁𝗵𝗿𝗲𝗲 (𝟰𝟴𝟯) 𝗙𝗶𝗻𝗮𝗹 𝗣𝗮𝘀𝘀𝗲𝗿𝘀 that the 𝗥𝗢𝗦𝗧𝗘𝗥 𝗗𝗘𝗧𝗔𝗜𝗟𝗦 section in your DMW EPS Account is now accessible.

𝗬𝗼𝘂 𝗮𝗿𝗲 𝗮𝗹𝗹𝗼𝘄𝗲𝗱 𝗼𝗻𝗹𝘆 𝗼𝗻𝗲 (𝟭) 𝘀𝘂𝗯𝗺𝗶𝘀𝘀𝗶𝗼𝗻. Please ensure that you read and understand all instructions and requirements thoroughly before proceeding.

1. 𝗦𝘂𝗯𝗺𝗶𝘁 𝘆𝗼𝘂𝗿 𝗺𝗲𝗱𝗶𝗰𝗮𝗹 𝗶𝗻𝗳𝗼𝗿𝗺𝗮𝘁𝗶𝗼𝗻 𝗶𝗻 𝗼𝗻𝗲 (𝟭) 𝗣𝗗𝗙 𝗙𝗶𝗹𝗲 𝗼𝗻𝗹𝘆.
2. If your medical clinic does not appear in the dropdown list, please select “Others.” The information will be updated accordingly.
2. 𝗜𝗳 𝗺𝗮𝗿𝗶𝘁𝗮𝗹 𝘀𝘁𝗮𝘁𝘂𝘀 𝗶𝘀 𝗠𝗮𝗿𝗿𝗶𝗲𝗱, 𝘆𝗼𝘂 𝗺𝘂𝘀𝘁 𝗽𝗿𝗼𝘃𝗶𝗱𝗲 𝗮 𝗱𝗶𝗳𝗳𝗲𝗿𝗲𝗻𝘁 𝗲𝗺𝗲𝗿𝗴𝗲𝗻𝗰𝘆 𝗰𝗼𝗻𝘁𝗮𝗰𝘁 𝗽𝗲𝗿𝘀𝗼𝗻.

4. 𝗞𝗼𝗿𝗲𝗮𝗻 𝗮𝘂𝘁𝗵𝗼𝗿𝗶𝘁𝗶𝗲𝘀 𝗱𝗼 𝗻𝗼𝘁 𝘂𝗻𝗱𝗲𝗿𝘀𝘁𝗮𝗻𝗱 𝗙𝗶𝗹𝗶𝗽𝗶𝗻𝗼 𝗹𝗮𝗻𝗴𝘂𝗮𝗴𝗲. Please translate the name of your school into 𝗘𝗻𝗴𝗹𝗶𝘀𝗵.
5. 𝗙𝗼𝗿 𝗵𝗶𝗴𝗵 𝘀𝗰𝗵𝗼𝗼𝗹 𝗴𝗿𝗮𝗱𝘂𝗮𝘁𝗲𝘀: Ensure your school name ends with “𝗛𝗶𝗴𝗵 𝗦𝗰𝗵𝗼𝗼𝗹” (ex. 𝙈𝙞𝙜𝙧𝙖𝙣𝙩 𝙃𝙞𝙜𝙝 𝙎𝙘𝙝𝙤𝙤𝙡).
𝗙𝗼𝗿 𝗰𝗼𝗹𝗹𝗲𝗴𝗲 𝗴𝗿𝗮𝗱𝘂𝗮𝘁𝗲𝘀: Ensure your school name ends with “𝗖𝗼𝗹𝗹𝗲𝗴𝗲” or “𝗨𝗻𝗶𝘃𝗲𝗿𝘀𝗶𝘁𝘆” (ex. 𝙈𝙞𝙜𝙧𝙖𝙣𝙩 𝘾𝙤𝙡𝙡𝙚𝙜𝙚 or 𝙈𝙞𝙜𝙧𝙖𝙣𝙩 𝙐𝙣𝙞𝙫𝙚𝙧𝙨𝙞𝙩𝙮).
For schools with “𝗜𝗻𝘀𝘁𝗶𝘁𝘂𝘁𝗲,” “𝗔𝗰𝗮𝗱𝗲𝗺𝘆,” or similar terms that do not include “High School” or “University/College” in their names, please ensure to put High School or University/College (ex. Migrant Institute High School, Migrant Academy High School, Migrant Institute University, Migrant Academy College).
6. 𝗔𝗹𝘁𝗲𝗿𝗻𝗮𝘁𝗶𝘃𝗲 𝗟𝗲𝗮𝗿𝗻𝗶𝗻𝗴 𝗦𝘆𝘀𝘁𝗲𝗺 (𝗔𝗟𝗦) and 𝗧𝗘𝗦𝗗𝗔 should 𝗡𝗢𝗧 be written as your school name. Indicate the actual school or institution where you completed the program.
7. 𝗡𝗖 𝗜𝗜 𝗶𝘀 𝗻𝗼𝘁 𝗲𝗾𝘂𝗶𝘃𝗮𝗹𝗲𝗻𝘁 𝘁𝗼 𝗮 𝘃𝗼𝗰𝗮𝘁𝗶𝗼𝗻𝗮𝗹 𝗴𝗿𝗮𝗱𝘂𝗮𝘁𝗲.
8. 𝗔 𝘃𝗼𝗰𝗮𝘁𝗶𝗼𝗻𝗮𝗹 𝗴𝗿𝗮𝗱𝘂𝗮𝘁𝗲 𝗶𝘀 𝗻𝗼𝘁 𝗰𝗼𝗻𝘀𝗶𝗱𝗲𝗿𝗲𝗱 𝗮 𝗰𝗼𝗹𝗹𝗲𝗴𝗲 𝗴𝗿𝗮𝗱𝘂𝗮𝘁𝗲.
