This clinic stopped replying.

A model that hallucinates 22 to 94 percent of the time is still confident every single time it answers.

That range comes from Stanford's 2026 AI Index Report, which tested leading models on an accuracy benchmark. For legal research specifically, the rate climbed to between 58 and 88 percent across major models.

That's the part most teams miss when they plug in an AI layer. The model does not know when it is wrong. It states a made-up answer with the same tone as the real one.

Guardrails are not a nice to have on top of an AI system. They are the difference between a tool that assists your team and one that quietly makes promises on your behalf.

Check your AI agents now. Are they built on multiple custom APIs?

A $440,000 government report was built on sources that never existed.

Deloitte Australia handed the report to the federal government in 2025. When researchers examined it, they discovered references to academic papers that were never written, along with a federal court quote that was never spoken. An Azure OpenAI model had been used to help produce it. After the issue came to light, the firm published a corrected version and agreed to refund part of the payment.

The worrying part isn't that AI invented something. It's that a fake reference looks just like a genuine one until someone takes the time to verify it. Making it look right was never difficult. Making sure it was right was.

If your AI-generated work is reaching a client, a regulator, or a court, a person still has to verify every source before it goes out. That step doesn't become optional just because the writing sounds confident.
