Most control failures are not documentation failures.
They are context failures.

The file exists.
The approval exists.
The timestamp exists.

What does not exist is the decision environment.

At the moment of action:

• What information was actually available?
• What constraints were in force?
• Who had authority at that point in time?
• What dependencies had not yet resolved?
• What was still unknown?

When those elements are not preserved contemporaneously,
review becomes reconstruction.

Reconstruction introduces data that did not exist at the time.
It changes the apparent reasonableness of the action.
It shifts evaluation from process fidelity to outcome bias.

That is where exposure is created.

Evidence can confirm that something happened.
Only context can demonstrate that it was permissible when it happened.

Organizations that treat decisions as conclusions
inherit hindsight.

Organizations that preserve decision environments
retain defensibility.

Control is not the archive.
Control is the preservation of conditions.

Each SDR operates within a clearly defined start and end time.
Once opened, the window cannot be extended or reframed.

2. Input Isolation
All formal inputs, constraints, and acknowledgments are captured without interpretation.
No recommendations, framing, or option‑shaping is permitted.

3. Contemporaneous Lock
The record is finalized before outcomes are known and cannot be edited afterward.

4. Mandatory Close‑Out
Each SDR ends with a formal closure.
There is no continuation, follow‑on advice, or scope expansion.
