🩺 Multi-Institutional Study on RAAS Inhibitor Use in Pediatric Lupus

Purpose:
Evaluate the potential of a pediatric learning health system to conduct large-scale, comparative effectiveness research in pediatric-onset systemic lupus erythematosus (SLE).
Focus on RAAS inhibitor use, treatment indications, and outcomes (hypertension & proteinuria).

Study design & population:
Data from 6 PEDSnet institutions.
1,303 pediatric SLE patients (457 with lupus nephritis, LN).
Control group: 200 non-SLE patients per site from the same clinics.

Key data metrics collected:
% of encounters with blood pressure (BP) readings.
% of BP readings paired with height (for accurate pediatric BP assessment).
% of patients with urinalysis within 7 days of SLE diagnosis.
Ability to classify proteinuria as normal/abnormal.
RAAS inhibitor prescription rates and timing.

Results:
BP recorded at 62% of encounters; 96% paired with a height.
Higher BP levels in SLE/LN patients vs. controls.
~1/3 of SLE patients diagnosed with hypertension.
60%-83% had urinalysis protein measurement within 7 days of diagnosis.
Proteinuria classification possible in 96% of tests; nearly all LN patients had ≥1 abnormal result
(94%-100%).
RAAS inhibitor use:
31% of SLE patients.
71% of LN patients (site range: 60%-84%).
90% of SLE cases starting RAAS inhibitors had hypertension, elevated BP, or proteinuria at
initiation.

Conclusions & implications:
Pediatric learning health systems can reliably capture key clinical data for lupus research.
Identified hospital-level variation in RAAS inhibitor use, signaling potential for treatment
standardization.
Supports more efficient multi-institutional comparative effectiveness studies in pediatric lupus.
