JOB DESCRIPTION:
Job Title: Perinatal Community Health Worker Work location: Madison, WI
Starting Date: February 3, 2025

POSITION SUMMARY: We seek a part-time perinatal Community Health Worker (PCHW) who will provide non-clinical health and social service support to Afghan women throughout their family planning journey. This includes education on prenatal, pregnancy, and post-natal care for both mothers and babies.

ROLES & RESPONSIBILITIES Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. • Ability to attend provided trainings both virtually and in person to be fully trained as a perinatal Community Health Worker • Work closely with the SunTalents Consulting and the UW-Madison research team to coordinate education, curriculum creation, and holistic care for clients. • Client note accurately within project databases and platforms • Coordination and delivery of group-based workshops for clients • Client needs assessments • Delivery of one-on-one services for clients • Provide interpretation for clients during educational sessions and events • Work alongside the UW-Madison research team to recruit eligible clients and assist with data collection efforts as needed for the project.

REQUIREMENTS: ● Mature and responsible person
● Desire to serve and empower marginalized and underserved communities
● High School diploma/GED. Work experience and lived experience may be considered in situations where education requirement is not met yet
● Fluent in Pashto/Dari and English spoken and written a must.
● Have familiarity with or strong ties to Afghan culture and the Afghan community in Madison, with ability to relate well to the experience of Afghan community members
● Clean driving record and personal vehicle for client in home visits and transportation support for medical appointments

● While performing the duties of this job, the employee is required to have ordinary ambulatory skills sufficient to visit other locations
● The ability to stand, walk, stoop, kneel, crouch, and manipulate (lift, carry, move) light to medium weights of 10-15 pounds.
● Requires good hand-eye coordination, arm, hand and finger dexterity, including ability to grasp, and visual acuity to use a keyboard, operate equipment and read application information.
● The employee frequently is required to sit, reach with hands and arms, talk and hear.

WORK ENVIRONMENT:
● Hybrid work environment with one physical office location – Madison – with some remote work allowed
● Great lengths of time working on computer, reading from computer screen, entering information, standing at copier or fax machine, and some time on the phone or in skype meetings may be required.
● Year-end archiving activities involve repeated lifting and bending.

● Equipment used: Employee computer (desktop or laptop), printer, and copier.
