Old code and clunky plugins add up too, especially on sites patched together over the years without a proper cleanup. Cheap hosting can also cap how fast your site will ever load, no matter what else you fix.
None of this usually needs a full rebuild. Most speed problems get fixed by cleaning up what's already there, often in a single afternoon of work, without touching your design at all.
