In nursing I was a band. In business, nobody gives a s**t what band I was.

The NHS gave me a ladder and I climbed it. Band 5, band 6, band 7, specialist qualifications on top. Twenty years of doing exactly what you are supposed to do in the order you are supposed to do it. I was good at it.

Here is what the top of that ladder looked like. More responsibility, more qualifications, and a rota that still decided when I saw my family and which December I was working. Every promotion bought me more accountability and none of my time back.

That is success. It is not freedom.

Business took the whole structure away. No band, no guaranteed wage, nobody deciding what I was worth. Which sounds liberating until you have to price something and there is nobody to ask. My first year I undercharged constantly, because in the NHS someone else set my value and out here I had no idea how to set my own.

But freedom without profit is just financial stress.

That is the part people leave out of the quit your job story. Quitting is not the achievement, anyone can quit. The achievement is building something that pays properly and gives you your time back. Both, not one. That is what I built Inject Digital for.

It is the same thing my clients are building. Most clinic owners I speak to are still doing shifts to fund the clinic. The goal was never a side income squeezed around a day job, it is a clinic profitable enough to make employment optional. That is a marketing problem and it is the one I solve.

Which brings me to next month. My registration expires in September and I have the hours to renew. Keeping it means keeping competence current, practice hours, CPD, prescribing. Real time out of the business for a job I am not going back to. Keeping it is the ladder, still there, still climbable.

Letting it go is backing the thing I already chose. I hit every goal I set for this business and I have never once stopped to notice. Maybe that is the actual decision.

One link in their profile used the same made-up review 192 times. It praised the company that sold the link.

That was the moment I knew.

I lost this clinic to someone cheaper earlier this year. Four months later I pulled their data, and I want to show you what cheaper actually bought.

443 websites suddenly linking to them. On a graph it looks brilliant.

Nine out of ten were built so Google ignores them. 434 of the 443 have no visitors at all, so no patient was ever going to arrive through one. And every single link pointed at the home page, not a treatment page. Nobody searches "aesthetic clinic". They search for the treatment they have already decided they want.

Meanwhile nothing was written. Not one new page in four months. Their best page is still a treatment comparison from back when somebody was writing, and it appeared in search nearly 90,000 times. One page, written properly, out-earned four months of buying links.

Result: rankings drifting backwards, traffic flat, diary unchanged.

Cheaper is not always wrong. But cheap SEO is nearly always links, because links are the one thing you can buy in bulk without knowing anything about clinics.

Three questions for your agency this month:
What did you publish for us?
How many people found us on Google, versus last month?
Show me the sites linking to us. Would a patient ever read one?

Vague answers mean you are paying for a graph.

The "What is Profhilo" blog had a good run. It's over.

AI answers those questions directly now, and almost nobody clicks through. Meanwhile the searches that are growing fastest are comparisons and planning questions, the moments people are actually deciding.

So stop writing definitions and start writing decisions: comparisons, real costs, suitability, who you'd turn away. That's where your clinical judgement shows, and it's what gets cited.

AI search has started doing something useful for serious clinics: it rewards proof of expertise and quietly ignores the anonymous ones.

It wants to know who performs the treatment, what they're qualified to do, who it suits, and what to expect. Most clinic sites answer none of that clearly.
