The AlterG Anti-Gravity Treadmill, based on science!

''When my knees are feeling good, I get in 25 to 30 running miles a week. When they are not happy, as is the case at the moment, I long for some kind of magical device that can help me recover.’’

There's no magic here, but my trainer has acquired a new rehab tool. The machine is called the AlterG Anti-Gravity Treadmill, which, he says is "the world's first and only treadmill using NASA-based anti-gravity technology."

So it's like you're into space, people!

This treadmill has an inflatable air chamber that surrounds the user's lower body and gently lifts the person off the treadmill.

With this assist, the user can walk or run at 20 to 99 percent less body weight, reducing impact on recovering joints. At a 20 percent setting, a 100kg person can train as if weighing 20kgs. At the 80 percent setting, that same runner trains as if weighing 80kg.

Post-operative patients can start partial weight-bearing exercise immediately.

Anti-gravity treadmills aren’t just for astronauts anymore – they’ve been adopted by professional athletes too, and now one of the machines is benefitting people at gym and therapy centre Hong Kong.

V2 a unique place that helps people with long-term, age-related and neurological conditions, has an AlterG treadmill, which uses air pressure to lighten the load of a person’s body weight by as much as 80 per cent.

Across the whole of Hong Kong there are only a few of the expensive machines, which were originally developed for NASA, but are now widely used for training and rehabilitation.

One of the first to try out the centre’s treadmill was a local man who has weakness in his right leg because of a stroke and foot drop.

He said: “It’s a totally different experience and certainly a different feeling. I’ve been coming here for years and I usually do 10 minutes on a ordinary treadmill – any more and my legs feel tired the next day.

“With the anti-gravity treadmill taking off 50 per cent of the weight I was able to go 27 minutes – almost three times as long as usual, and no problem at all.

“A gentleman came after me who had had a severe stroke and could hardly walk. You could see a definite improvement – his legs were moving in a walking motion, and he was doing it on his own.

“I’d think it would give a great benefit to people like him who are more incapacitated than me.

“I think also, for people who can’t walk very well, the feeling of putting one foot in front of the other would give them a lot of confidence.”

AlterG Anti-Gravity Treadmill - Speed work, rehab and more

Have you heard about the treadmill that can make you weightless?

Nope it’s not a magic diet pill, but for many injured athletes, amputees and military vets it has become one of the best recovery tools available. It allows anyone with an injury to resume or continue activity at their previous level of intensity with reduced impact to hips, knees and ankles.

“The Alter G is the most significant advance in training equipment for Rehab in the last half century.”

''A few months ago when my knee pain began, I had a chance to test one out and learn more about this fancy, and very expensive, machine. I was literally swept off my feet (come on that’s funny!) so I wanted to share more about this tool to see if it’s right for your rehab, training or even speed work!

HOW IT CAME ABOUT
In the 90’s, NASA began looking for a pressurized method that would allow astronauts to maintain fitness during space missions because without the movement they were at risk for bone mass and muscle loss.

The result was the anti-gravity treadmill, now sold as the AlterG.

At a cost of £35,000 to £55,000 the Alter G is found in some gyms, hospitals, physios or University sports facilities, as opposed to say my home gym.

WHO USES THE ALTER G?
During a test run of the treadmill, I learned some great information from Dan about the wide variety of uses for the AlterG:

Amputees learning to move more quickly on their new limb

Injured soldiers utilize it during rehabilitation

Seniors needing to work on strength and balance

Assists people with traumatic brain injuries in relearning gait

Elite athletes looking to increase foot turnover without the impact

Athletes returning from certain injuries or wanting to increase workouts with less impact

The AlterG is a chance to practice increased walking speed or running without the fear of falling, which can be an issue for seniors and new amputees. It allows them to practice the movements and slowly gain confidence in their ability to move, by increasing the body weight on the treadmill until they are at 100%.

HOW DOES IT WORK?
AlterG’s treadmill looks like something from the future…a vacuum sealed bubble encompassing your lower body and the treadmill belt.

After getting a quick once over of the machine, Dan will walk you through the set up process. Since I was new to the machine they gave me a pair of tight neoprene shorts, which you pull up right over your shorts (wear spandex!).

The shorts have small skirt around the waist and after stepping in to the center of the machine you pull up the bars which have the opposite size of a zipper.

Once zipped, you’ve completed the “bubble”, aka the seal which will now allow you to see what it’s like to run with less and less gravity! The treadmill measures your weight (but doesn’t shout it to the world, no worries) and then using this information the machine calibrates what is 100% pressure for your body. In other words, normal running.

Depending upon your reason for using the machine, you might start out with a walk at 100% or go immediately to a reduce weight feeling by lower the percentage. At 20% the machine is fully supporting your body and your legs mostly feel like they are free flying through the air.

The machine can make you feel up to 80 percent lighter—a basic example would be someone who weighs 100kg, could feel as light as 20kg on the treadmill.

The the machine uses “unweighting technology” to remove pressure from your body and make it feel as though you are running on air. Using controls similar to incline, you can decrease the amount of weight on your body.

You don’t feel like the machine is raising and lowering you, rather you notice the weight on your feet becoming more or less.

Due to the neoprene shorts, you’re going to work up a hot sweat too! Great to work on my 'thigh gap'!

BENEFITS OF WEIGHTLESS RUNNING
I loved hearing some of the amazing stories Dan was able to share with me about patients who returned to a complete active life after being able to use this machine post injury.

On the other end of the spectrum are the elite athletes who have begun to gravitate (pun intended) to the AlterG.

For the elite endurance runner a frequent concern is the impact of pounding the pavement for so many miles every single week. This machine allows them to maintain mileage on easy runs with a slight relief on their joints. - It was reported in 2011 that Paula Radcliffe used her own and when Mo Farah used it in 2012 to take double gold at the Olympics.

'Mature' Pro Footballers looking to increase their speed have found this tool allows them to replicate a much faster leg turn over for a longer period because it doesn’t stress their heart as heavily

And Pro Rugby players returning from injury are able to more quickly return to their old levels of fitness by starting with workouts on the AlterG.

Athletes can practice a better range of motion by reducing body weight, the legs will naturally extend farther. Practicing better alignment and technique without fear of injury.

So would do I use it? Well, doing my Long Low Heart Rate training is fab and it's phenomenal to work on speed here and see if I could still keep my rate lower due to the ease of the movement.

Could an Anti-Gravity Treadmill save your damaged knees?

My fall came during a game of netball and, though I didn't hear a snap of bone or the ping of a tendon, my right leg stopped working immediately.

Putting weight on it felt like walking on cooked spaghetti. The next day, my knee had swollen and seized up. My husband carried me into a walk-in centre (how ironic), and an hour later I left on crutches. The verdict was that I had strained a ligament but it would get better soon.

It wasn't until five months later that I learnt I'd ruptured my anterior cruciate ligament (ACL). The knee had improved, to a degree, but still kept giving way, so I was sent for an MRI scan.

Telling my knee surgeon, Jonathon Lavelle of the Fortius Clinic in London, how many weddings I'd been to, semi-inebriated, and dancing on five-inch heels drew a look of abject horror. Each time, he said, I'd been risking far greater damage and shredding what few ligaments were left holding my knee together.

He booked me in for reconstructive surgery, which entailed removing a 20cm length of hamstring and looping it through my knee. For the next six months, I would need careful rehabilitation – oh, and anti-gravity treatment.

So it was that after three months of physiotherapy and wobbling about on the cross-trainer, I stepped on to an AlterG Anti-Gravity Treadmill, a machine normally used by elite athletes such as Mo Farah and Gareth Bale to recover from serious injury. Paula Radcliffe, who bought a £36,000 device, said it was one of the best investments she has ever made. Now, Gordon Ramsay is using one to get over an Achilles tendon problem, Stephen Fry has had a go, and Burnley and Middlesborough are the only Premiership football teams not to own one.

The premise is simple. Users are zipped into a pair of specialist neoprene shorts and a waist-high airtight bag which encloses the treadmill.

The space around your legs fills with air, lifting you up, allowing you to exercise at a fraction of your body weight and reducing impact and pressure on your joints as your run.

“It's like being a cork in a bottle of champagne,” explains the physio as I run on one of their machines. “It uses air pressure. The more air pressure you get, the more upward lift you get. The pressure can be 1.1 pounds per square inch (psi), so it's like roughly going waist-deep into water. Very accurately, you can take someone down from 100 per cent of their body weight to just 20 per cent - the same feeling as walking on the moon - in one per cent increments.”

Developed by NASA in the nineties, it was originally designed to create a downward force and allow astronauts to exercise in space to prevent muscle atrophy. But deemed too bulky for space, the idea was discarded until someone redesigned the model by flicking the air pressure controller to decrease the feeling of gravity, rather than add to it.

Marathon runners like it because it allows them to train longer without risking injury, and those in recovery, like me, can get moving more quickly.

Now, the NASA-patented technology is being rolled out into rehabilitation centres, NHS wards, private clinics and gyms, to be used with a range of serious conditions including arthritis and osteoarthritis, spinal injuries and neurological conditions such as Parkinson's and multiple sclerosis. A video clip of a man who had been in a wheelchair for five years but, with the help of the AlterG treadmill, was able to jog again, has been watched by almost five million people.

The treadmills are also being used in the fight against need hip, knee and ankle replacements.

“While this can't make that joint any better, once you've got a degenerative knee, this can strengthen the muscle around the knee,” my Physio says. “It gets the fluid working around the joints and can delay surgery by years.”

Overweight or obese patients are able to run without joint pain and experience weight loss at their target weight, a dramatic motivational tool. Only those with untreated deep vein thrombosis or an unstable fracture are advised to stay off the machines for fear of doing further damage.

Dan Fivey, a sports therapist at from the UK who has owned an AlterG for 10 years, has witnessed patients in tears using the machine, so overjoyed are they by the feeling of weightlessness on their battered joints and having been offered a way to get fitter.

“Many people who are in pain with arthritis in their knees, ankles and hips stop doing exercise altogether,” says Fivey. “With this, they can keep moving without aggravating their joints and worsening the condition. It maintains muscle tone, keeps the heart and respiratory system working and is good psychologically, when immobile people tend to get a bit depressed. Coming in here and using the machine gives them a real boost.”

It's no exaggeration to say that for one of Fivey’s patients, Bob, a 71-year-old retired teacher from Cheltenham , using the AlterG was life-changing. Bob had a cardiac arrest and had suffered from arthritis in his feet and knees for years. His wife, had also developed ovarian cancer.

“Even though she had this horrible disease, she was more mobile that I was,” he says bleakly at the memory. Bob was unable to go out or walk far without severe discomfort. “I felt as though I was holding her back. I needed to be fit to look after her.”

Bob started with short 15-minute strolls on the treadmill at 20 per cent of his full body weight to build muscle strength and endurance. Today, he manages three miles twice a week at 75 per cent of his weight, a distance that takes him half an hour, and has restored his legs. He is able to go on walking holidays with his wife, who has recovered, again and they've just returned from a trip to the Scilly Isles where he walked for up to five hours a day.

“We don't climb mountains, but I can manage uneven surfaces. It has been brilliant. Without it, I don't want to think where we'd be. I certainly wouldn't be able to enjoy the lifestyle I have.”

I've been on the anti-gravity treadmill for half an hour and, even after ten months without exercise, there is not a bead of sweat on me. My breath is coming easily and running feels fluid, light and loose. Usually, when I up the pace – to jog for a bus – my leg aches and the knee joint feels slightly unstable. Now, I notice neither. I could be running on air, which, essentially, I am.

It's a great feeling. But I'd try anything – even neoprene shorts – to stay off the crutches.
