"We need an NPS report by segment" specifies an output. "We need to understand why our automotive renewal rate dropped from 87% to 71%, and what to change to recover it" specifies a decision.

The most common reason: the solution came before the diagnosis. A kaizen event gets designed around a hypothesis about where the problem lies and if that hypothesis is wrong, you get activity without impact.

Real operational diagnostics starts by asking the workers who actually live the process: where time is lost, where rework happens, where handoffs fail. Then you cross-check that against the hard data, cycle times, scrap rates, downtime logs. Only then do you design the fix.
