A U.S. Air Force RQ-4B Global Hawk assigned to the 348th Reconnaissance Squadron at Grand Forks Air Force Base was lost during a long-range overseas ferry mission after suffering a sudden in-flight engine failure. *
Investigators later determined that a cracked accessory gearbox oil line - caused by high-cycle metal fatigue - led to rapid oil loss and a complete engine shutdown during flight.
With limited recovery options available, the mission crew directed the aircraft away from populated areas and safely ditched it into the ocean near Naval Station Rota, Spain. No injuries or damage on the ground were reported.
The official investigation also noted that the approved mission routing provided insufficient emergency divert airfields for the unmanned aircraft, reducing the crew's available options during the emergency.
The RQ-4B Global Hawk and its reconnaissance payload, valued at nearly $99 million, were destroyed in the incident.
The mishap highlighted how a relatively small mechanical failure can quickly escalate into the loss of a high-value strategic surveillance aircraft during long-duration operations.

Ground crews suddenly noticed sections of the wing skin warping and buckling under the extreme stress, forcing an immediate halt to operations. Although the incident did not trigger a fire or violent explosion, the damage was so severe that the aircraft was ultimately removed from service.
The massive C-141 relied on integrated fuel cells housed inside the wings, requiring highly controlled fuel flow, venting, and pressure management during every refueling cycle. Even a small error in the sequence or monitoring process could place enormous strain on the aging airframe.
Following the incident, investigators determined that procedural mistakes and insufficient pressure monitoring contributed heavily to the failure. The event became another powerful reminder throughout military aviation that ground operations can be just as dangerous as flight itself, especially when handling large aircraft with complex fuel systems.

Staff Sgt. Sky R. Mote, 27, died Aug. 10 while conducting combat operations in Helmand province, Afghanistan alongside Captain Matthew P. Manoukian and Gunnery Sergeant Ryan Jeschke. He was assigned to 1st Marine Special Operations Battalion. He was awarded the Purple Heart, the Navy-Marine Corps Commendation Medal, a Navy-Marine Corps Achievement Medal, two Combat Action Ribbons and three Good Conduct Medals.

Bradley Allan Kasal (born May 1, 1966) is a United States Marine who received the Navy Cross for heroic actions performed as the first sergeant of Weapons Company, 3rd Battalion, 1st Marines during a firefight in Operation Phantom Fury in Fallujah, Iraq on November 13, 2004. A 23-year-old flight instructor was killed in a plane crash at a Virginia airport that was being piloted by an aviation student, police say.
The crash was reported Thursday afternoon just after 3 p.m. at the Newport News Williamsburg
International Airport.

Hanover, Maryland was piloting the plane - a single engine Cessna 172 - during take-off, The steep angle of the take-off caused the plane's engine to stall, officials say, and sent the aircraft into a dive. The plane crashed from a height of about 100 feet into an embankment near the runway.
Investigators say Viktoria Theresie
Izabelle Ljungman - the flight instructor and the licensed commercial pilot aboard the plane - was killed in the crash. Ljungman was from Williamsburg, Virginia.
Oyebode and another 18-year-old male passenger were students in an aviation class. They were both transported to Riverside Regional Hospital with life threatening injuries.
The investigation is still continuing at this time.
