A U.S. Air Force RQ-4B Global Hawk assigned to the 348th Reconnaissance Squadron at Grand Forks Air Force Base was lost during a long-range overseas ferry mission after suffering a sudden in-flight engine failure. *
Investigators later determined that a cracked accessory gearbox oil line - caused by high-cycle metal fatigue - led to rapid oil loss and a complete engine shutdown during flight.
With limited recovery options available, the mission crew directed the aircraft away from populated areas and safely ditched it into the ocean near Naval Station Rota, Spain. No injuries or damage on the ground were reported.
The official investigation also noted that the approved mission routing provided insufficient emergency divert airfields for the unmanned aircraft, reducing the crew's available options during the emergency.
The RQ-4B Global Hawk and its reconnaissance payload, valued at nearly $99 million, were destroyed in the incident.
The mishap highlighted how a relatively small mechanical failure can quickly escalate into the loss of a high-value strategic surveillance aircraft during long-duration operations.
