Please be aware of scammers targeting senior citizens, below are a few scams to be aware of:

Top scams targeting seniors

1. Medicare/health insurance scams
- Perpetrators may pose as a Medicare representative to get senior to provide their personal information (SSN #, Medicare #, Date of Birth)

2. Identity theft
- “We need to verify your account number”
- May ask for Social Security number or credit card information
- Can find personal information in trash

3. Fake charities
- Usually done by telephone
- Scammers use respected organizations or recent disasters

A Patient Centered Medical Home is a model of care that emphasizes care coordination and communication to transform primary care into “what patients want it to be.”

A PCMH puts patients at the forefront of care by building better relationships between people and their clinical care teams. Research shows that PCMHs improve quality, reduce costs and improve both practice staff and patient satisfaction. The National Committee for Quality Assurance (NCQA) helps primary care practices transform into PCMHs through its recognition program.
