This is what my standard-yet-simple MU booking form looks like/the type of questions I ask during the "Estimate"-phase of a booking enquiry. (There's also a hair one, and an altogether different one for bridal parties, fyi.)
This (along with inspo pics/ref images) helps me to get a better idea of what we're envisioning, and pre-empting troubleshooting/planning time accordingly.
